ABSTRACT

OBJECTIVE@#To explore clinical effect of arthroscopic meniscus tear strapping suture by rotator cuff suture threader.@*METHODS@#Forty patients with meniscus tear injury admitted from July 2015 to May 2019, including 27 males and 13 females, aged from 20 to 55 years old with an average of (36.0±1.4) years old. Menisci laceration was sutured with rotator cuff suture thread under arthroscopy. Postoperative complication was observed, Lysholm knee joint score before and after operation at 12 months were used to evaluate clinical effects, visual analogue scale (VAS) and range of knee flexion and extension were applied to evaluate recovery of pain and function.@*RESULTS@#All patients were followed up from 12 to 15 months with an average of (12.6±0.7) months.No complication such as joint effusion, suture failure occurred. Two patients occurred mild pain after activity without clinical physical abnormality, and 1 patient manifested moderate pain with joint space tenderness, the other rest without abnormal. Lysholm knee joint score was increased from (49.55±1.21) preoperatively to (98.95±0.42) at 12 months after operation, VAS score decreased from (5.18±0.78)preoperatively to (1.03±0.77) at 12 months after operation, and range of knee joint flexion and extension activity increased from (50.63±9.20)°preoperatively to (130.38±4.99)°after operation, and there were statistical differences in Lysholm knee joint score, VAS and range of knee joint flexion and extension activity (@*CONCLUSION@#Arthroscopic strapping suture by rotator cuff suture threading device applies to most meniscus injuries, including medial meniscus posterior horn tears, lateral meniscus body tears and lateral meniscus posterior horn tears. This technique meets the need of full-internal meniscus suture without specialmeniscus suture, and has advantages of convenient operation, less complications and good postoperative function.

ABSTRACT

Introducción: La dehiscencia anastomótica (DA) es una complicación de la cirugía colorrectal que determina alta morbimortalidad y peor pronóstico oncológico. El objetivo de este estudio es analizar su relación con factores de riesgo y describir su manejo terapéutico. Métodos: Estudio retrospectivo, observacional y analítico. Se incluyeron resecciones colorrectales de coordinación por patología neoplásica maligna con anastomosis primaria, entre enero de 2015 y noviembre de 2018. Resultados: Ciento doce pacientes cumplieron con los criterios de inclusión. La tasa de DA fue 17% y la mortalidad 15%. El tamaño tumoral se asoció de forma estadísticamente significativa con un mayor riesgo de falla de sutura. Discusión: Nuestros resultados son similares a los reportados en la literatura, destacándose una alta proporción de DA tempranas y por lo tanto de reintervención y detransitación. Conclusiones: El manejo de la DA es un reto terapéutico y debe ajustarse a muchos parámetros, siendo uno de los más importantes la experiencia del cirujano. (AU)


Introduction: Anastomotic dehiscence (AD) is a complication of colorectal surgery that determines high morbidity and mortality and a worse oncological prognosis. The objective of this study is to analyze its relationship with risk factors and describe its therapeutic management. Material and Methods: Retrospective, observational and analytical study. Colorectal resections with primary anastomosis for malignant neoplastic pathology performed between January 2015 and November 2018 were included. Results: One hundred and twelve patients met the inclusion criteria. The AD rate was 17% and mortality 15%. Tumor size was statistically significantly associated with an increased risk of AF. Discussion: Our results are similar to those reported in the literature, highlighting a high proportion of early AD and therefore of re-intervention and intestinal diversion. Conclusions: The management of AD is a therapeutic challenge, and must be adjusted to many parameters, one of the most important being the surgeon's experience. (AU)

ABSTRACT Corneal ectasia is one of the main complications of keratorefractive procedures. In this report, we describe a case of corneal ectasia after laser-assisted in situ keratomileusis), which progressed with acute hydrops and aqueous leakage and required a suture for correction.


RESUMO A ectasia corneana é uma das principais complicações das cirurgias refrativas. Neste caso, descrevemos um caso de ectasia corneana induzida por laser-assisted in situ keratomileusis, que evoluiu com hidrópsia aguda e extravasamento de humor aquoso, necessitando de sutura corneana.

RESUMEN: El objetivo de este estudio fue comparar los hilos de sutura de seda negra y la sutura nylon con respecto a la adhesión bacteriana post extracción dental simple de piezas posteriores. El presente trabajo se desarrolló en la Clínica Odontológica y el Laboratorio de Microbiología de la Unidad de Ciencias Biomédicas y Biotecnología de la Universidad Privada Antenor Orrego (Trujillo, Perú), entre agosto y setiembre de 2014. Se trabajó con 19 unidades de muestreo para hilo de seda negra y 19 para nylon. El procedimiento de exodoncia para todos los pacientes fue estandarizado. Luego de la extracción, se procedió a suturar la herida quirúrgica, colocando dos puntos circunferenciales, uno con cada tipo de hilo. A los 7 días, los hilos fueron retirados y sometidos a análisis microbiológico. La adhesión bacteriana de los hilos de sutura seda negra y nylon fue comparada empleando el test de Wilcoxon. Además la adhesión bacteriana de cada hilo fue comparada con la adhesión en hilos estériles (control) empleando el test de Mann-Whitney para una p<0,005. Al comparar la adhesión bacteriana de ambos hilos, se encontró diferencia entre ellos (p=0,027), correspondiendo mayor adhesión al hilo de nylon. La adhesión bacteriana fue mayor en el hilo de sutura de nylon comparado con el hilo de sutura de seda negra.


ABSTRACT: The objective of the study was to compare black silk suture threads and nylon suture with respect to bacterial adhesion after simple dental extraction of posterior pieces. The present study was developed in the Clínica Estomatológica and the Laboratorio de Microbiología of the Unidad de Ciencias Biomédicas y Biotecnología de la Universidad Privada Antenor Orrego (Trujillo, Peru), between August and September 2014. We worked with 19 units sampling for black silk thread and 19 for nylon. The procedure of exodontia for all patients was standardized. After the extraction, the surgical wound was sutured, placing two circumferential points, one with each type of thread. After 7 days, the threads were removed and subjected to microbiological analysis. The bacterial adhesion of the black silk and nylon suture strands was compared using the Wilcoxon test. In addition, the bacterial adhesion of each strand was compared with the adhesion in sterile strands (control) using the Mann-Whitney test for p <0.005. When comparing the bacterial adhesion of both threads, a difference was found between them (p = 0.027), corresponding greater adhesion to the nylon thread. Bacterial adhesion was higher in nylon suture compared to black silk suture.

Introducción: A pesar de que la evidencia actual respalda ampliamente el abordaje laparoscópico para las resecciones de colon y recto en función de su seguridad y resultados oncológicos, el desarrollo y adopción del mismo ha sido lento y aún hoy no es de rutina en la mayoría de los centros. Se presenta la experiencia inicial en la Clínica Quirúrgica 2 del Hospital Maciel. Métodos: Estudio retrospectivo que comprende 41 resecciones laparoscópicas en el período 2016-2018. Resultados: El tiempo operatorio medio fue de 3 h para las colectomías y 4 h para las resecciones de recto. El porcentaje de conversión fue de 10%, la falla de sutura del 12% y la mortalidad a 30 días del 13%. Discusión: El abordaje laparoscópico para la colectomía y resección de recto es seguro y con aceptables resultados oncológicos de acuerdo a nuestros resultados. La curva de aprendizaje se sustenta en el volumen anual de cirugías, la realización de otros procedimientos de laparoscopía avanzada y el entrenamiento en simuladores. Conclusiones: nuestra experiencia muestra resultados inmediatos similares a los reportados en la literatura nacional, aunque el tiempo seguimiento aún es insuficiente para analizar los resultados oncológicos a largo plazo.


Introduction: Although current evidence widely supports the laparoscopic approach for resections of the colon and rectum according to their safety and oncological results, the development and adoption of the same has been slow and even today is not routine in most centers. The initial experience is presented in the Surgical Clinic 2 of the Maciel Hospital. Methods: A retrospective study comprising 41 laparoscopic resections in the period 2016-2018. Results: The mean operative time was 3 hours for colectomies and 4 hours for rectal resections. The conversion rate was 10%, the suture failure was 12% and the 30-day mortality was 13%. Discussion: The laparoscopic approach for colectomy and rectal resection is safe and with acceptable oncological results according to our results. The learning curve is based on the annual volume of surgeries, the performance of other advanced laparoscopy procedures and training in simulators. Conclusions: our experience shows immediate results similar to those reported in the national literature, although the follow-up time is still in sufficient to analyze the long-termoncological results.


Introdução: Embora as evidências atuais apóiem amplamente a abordagem laparoscópica para ressecções do cólon e do reto de acordo com sua segurança e resultados ontológicos, o desenvolvimento e a adoção dos mesmos têm sido lentos e até hoje não é rotineiro na maioria dos casos os centros. A experiência inicial é apresentada na Clínica Cirúrgica 2 do Hospital Maciel. Métodos: Estudo retrospectivo com 41 ressecções laparoscópicas no período 2016-2018. Resultados: O tempo operatório médio foi de 3 horas para colectomias e 4 horas para ressecções retais. A taxa de conversão foi de 10%, a falha na sutura foi de 12% e a mortalidade em 30 dias foi de 13%. Discussão: A abordagem laparoscópica para a colectomia e ressecção retal é segura e com resultados ontológicos aceitáveis de acordo com nossos resultados. A curva de aprendizado é baseada no volume anual de cirurgias, no desempenho de outros procedimentos avançados de laparoscopia e no treinamento em simuladores. Conclusões: nossa experiência mostra resultados imediatos semelhantes aos relatados na literatura nacional, embora o tempo de seguimento ainda seja insuficiente para analisar os resultados ontológicos em longo prazo.

Introdução: A mamoplastia de aumento é o procedimento cirúrgico estético mais realizado nos EUA e o segundo no Brasil. Simastia é uma de suas possíveis complicações. Esta é definida como o mal posicionamento medial dos implantes mamários, que cruzam a linha média, com perda do sulco intermamário. Métodos: Trabalho tipo série de casos. Duas pacientes foram submetidas à correção de simastia pósmamoplastia de aumento. A via de acesso utilizada foi a incisão inframamária prévia. A correção do sulco intermamário foi realizada por escarificação das superfícies anterior e posterior da cápsula, associada a pontos de adesão com fio monofilamentar não absorvível (4 linhas com 6 pontos cada, compreendendo uma faixa vertical de aproximadamente 4 cm de largura na região pré-esternal). Em um dos casos o implante foi recolocado imediatamente e, no outro, em procedimento realizado 3 meses após. Resultados: Obtivemos um novo sulco intermamário satisfatório, com adequado resultado estético em ambos os casos. Não se observou recorrência após 9 e 11 meses de observação. Conclusões: A abordagem com suturas de adesão para a redefinição do sulco intermamário foi efetiva para o tratamento da simastia pós-mamoplastia de aumento.


Introduction: Augmentation mammoplasty is the most commonly performed esthetic surgical procedure in the United States and second in Brazil. Symmastia is one of its possible complications. This is defined as incorrect medial positioning of the breast implants, which cross the midline, losing the intermammary sulcus. Methods: Work type, series of cases. Two patients underwent symmastia repair after breast augmentation. The access route used was the previous inframammary incision. The repair of the intermammary sulcus was performed by scarification of the anterior and posterior capsule surfaces, associated with adhesion suture with non-absorbable monofilament threat (4 lines with 6 points each, comprising a vertical band of approximately 4 cm wide in the presternal region). In one of the cases, the implant was immediately replaced, and in the other, the procedure was done 3 months later. Results: In both cases, we obtained a new satisfactory intermammary sulcus, with proper esthetic result. No recurrence was observed on follow-ups at 9 and 11 months. Conclusions: The approach with adhesion sutures to redefine the intermammary sulcus was effective in the treatment of symmastia after breast augmentation.

ABSTRACT Introduction Penile curvature (PC) can be surgically corrected by either corporoplasty or plication techniques. These techniques can be complicated by post-operative: penile shortening, recurrent PC, painful/palpable suture knots and erectile dysfunction. Objective To avoid the complications of corporoplasty and plication techniques using a new technique: combined plication-incision (CPI). Materials and Methods Two groups (1&2) were operated upon: group 1 using CPI and group 2 using the 16-dot technique. In CPI, dots were first marked as in 16 dot technique. In each group of 4 dots the superficial layer of tunica albuginea was transversely incised (3-6mm) at the first and last dots. Ethibond 2/0, passed through the interior edge of the first incision plicating the intermediate 2 dots and passed out of the interior edge of the last incision, was tightened and ligated. Vicryle 4/0, passed through the exterior edges of the incisions, was tightened and ligated to cover the ethibond knot. Results Twelve (57.1 %) participants in group 2 complained of a bothering palpable knot compared to none in group 1 with statistically significant difference (P=0.005). Postoperative shortening (5mm) of erect penis, encountered in 9 participants, was doubled in group 2 but with insignificant difference (P>0.05). Post-operative recurrence of PC, was encountered in only 1 (4.8%) participant in group 2, compared to none in group 1, with insignificant difference (P>0.05). Post-operative erectile rigidity was normally maintained in all participants. Conclusion The new technique was superior to the 16-dot technique for correction of PC.

ABSTRACT

Abstract: Background: Surgical sutures, wound tension, additional skin incisions and other factors may result in recurrence of tumor-like scar. Objective: To investigate the role of wound natural healing therapy in tumor-like hypertrophic scar. Methods: In this study, tumor-like hypertrophic scars of 47 cases were excised completely and the residual wounds were treated with natural healing. The short-term and long-term effects of treatment were evaluated. Results: All cases were successfully cured by natural healing therapy. The healing time of the maximum wound (80mm × 20mm) and the minimal wound (5mm× 5mm) was 25 days and 7 days respectively. The size of new skin scars ranged from 3mm to 11 mm. Clinical followed-up was performed in 34 cases for 36 months. Among them, no recurrence happened in 31 cases and new scar size ranged from 2mm to 8mm, while local recurrence happened in 3 cases whose scar size were less than 5 mm. Study Limitations: The cure rate of the therapy was 91.2%. Conclusion: The wound natural healing therapy is effective in treating tumor-like hypertrophic scar, which can prevent recurrence and has good cosmetic results.

Introdução: Cicatrização de ferida é um processo bem organizado que tem como finalidade a reparação tecidual completa. Colas e adesivos tópicos oferecem uma alternativa não invasiva, de retirada fácil e espontânea; boa força tênsil; menor tempo de aplicação, sendo o Prineo® uma cola adesiva associada a uma malha de poliéster aplicados sobre a ferida. Método: Trata-se de um estudo transversal retrospectivo com análise de 101 procedimentos cirúrgicos no período de 2012 a 2014, nos quais a síntese da ferida operatória ocorreu com fios cirúrgicos de náilon ou Prineo®, sendo aplicada análise estatística. Resultados: Neste estudo, seis pacientes apresentaram dermatite de contato ao uso do Prineo® com significância estatística (p = 0,042). O uso desse sistema diminuiu a taxa de alargamento cicatricial (p < 0,05). O presente trabalho não apresentou diferença estatística (p = 0,068) na qualidade da cicatriz entre os pacientes que utilizaram Prineo® em relação ao grupo controle, demonstrando que em ambos os grupos o resultado cicatricial foi de excelente (87%) a bom (27%) na sua maioria. Conclusão: Conclui-se no estudo que os pacientes que utilizaram o sistema de octil-2-cianocrilato associado a malha, Prineo®, apresentaram menores índices de alargamento cicatricial, dependentes de uma espessura de derme satisfatória, e maiores taxas de dermatites por contato em relação àqueles em que a ferida foi encerrada com fios cirúrgicos. Os dois grupos demonstraram qualidades cicatriciais excelentes a bons, sem diferença estatística em tais resultados estéticos cicatriciais.


Introduction: Wound healing is a well-organized, directed process of tissue repair. The process can be expedited using topical glues and adhesives, which offer a non-invasive, easily removable alternative to suturing. Furthermore, such products have good tensile strength and involve lower application time. In particular, the Prineo® adhesive is applied to a polyester mesh that covers the wound. Method: We carried out a retrospective, cross-sectional study, with subsequent statistical analysis , involving 101 surgical procedures in which wound closure was performed using either nylon sutures or Prineo®. All the procedures were performed between 2012 and 2014. Results: Six patients had contact dermatitis after Prineo® was used, with statistical significance (p = 0.042). Furthermore, Prineo® decreased the rate of scar enlargement (p < 0.05) . There was no statistical difference between the Prineo® and suture groups in terms of scar quality (p = 0.068); in both groups, the scar result was mostly excellent (87 %) to good (27%). Conclusion: Patients whose wounds were closed using Prineo® a system involving octyl-2-cyanoacrylate and an associated polyester mesh displayed lower rates of scar enlargement, which depended on whether the thickness of the dermis was satisfactory. However, the same patients had higher rates of contact dermatitis than those whose wounds were closed using surgical sutures. Both groups showed excellent to good scar quality, with no significant difference in terms of esthetic scar results.

ABSTRACT PURPOSE: To develop an experimental model for incisional hernias and to compare morphological and functional aspects of hernia repairs by suture, polypropylene mesh and collagen mesh. METHODS: A defect measuring 7cm x 2cm was created in the anterior abdominal of 28 New Zealand male rabbits, divided into four groups (n = 7): (1) control, (2) suture of the anterior sheath of the rectus abdominal muscle, (3) setting of polypropylene mesh, and (4) setting of collagen mesh. On the 90th postoperative day, the animals were examined to verify the presence of incisional hernia. Samples of abdominal wall and scar were collected for histological study. RESULTS: Incisional hernia was identified in 85.7% of the control group, 57.1% of the suture group, 42.9% of the collagen mesh group, and none in the polypropylene mesh group (p = 0.015). Mesh exposure could be identified in 71.4% of the animals in group 3 and in no animal in group 4 (p = 0.021). The polypropylene mesh is effective in the treatment of abdominal wall defects, causing an intense inflammatory reaction. CONCLUSION: The collagen mesh is biocompatible, producing a minimal inflammatory reaction, but fails in the treatment of abdominal wall defects.

ABSTRACT

Relato do caso de um paciente do sexo masculino, 44 anos, que deu entrada pelo Serviço de Emergência, no dia 17 de novembro de 2013, vítima de acidente com disco de Makita, apresentando ferida extensa em toda face dorsal do primeiro quirodáctilo esquerdo e exposição tendínea. Demonstra a utilização de sutura elástica, em atendimento inicial na urgência, e a possibilidade de tratamento de ferida traumática com perda de substância, com resolução satisfatória em único tempo cirúrgico. A opção cirúrgica apresentou resultado funcional adequado e preservação estética.


This is a case report of a 44-year-old male patient who was admitted to the emergency room on November 17, 2013, after an accident with a Makita Disc. The patient presented with an extensive wound throughout the dorsal surface of the left thumb, with tendon exposure. The use of elastic suture is demonstrated as the initial care in the emergency department. We present satisfactory resolution in a single surgical procedure of traumatic wounds in a patient presenting with loss of tissue. The chosen surgical approach presented adequate aesthetic result and functional preservation.

ABSTRACTPurpose:To assess patient characteristics, risk factors, outcomes, and the treatment of wound dehiscence (WD) in patients after corneal keratoplasty.Methods:Retrospective chart review of 11 eyes of 11 patients with corneal grafts who underwent repair of WD from January 1, 2004 to December 31, 2012 at Hospital Oftalmologico de Brasilia.Results:Eight (72.7%) patients were men and three were women. Six (54.5%) patients had deep anterior lamellar keratoplasty (DALK) and 5 had penetrating keratoplasty. The mean age at trauma was 31.1 years. The mean time from corneal keratoplasty to WD was 12.82 months (range, 3-33 months). The mean best-corrected visual acuity of patients before trauma was 20/60 (0.48 logMAR) and after final treatment was 20/160 (0.90 logMAR) (P=0.15). In one case, visual acuity decreased to no light perception because of retinal detachment and phthisis bulbi. Accidental blunt trauma and fall were the most common causes of WD.Conclusion:Patients who undergo corneal keratoplasty have a life-long risk of WD. The full-thickness rupture at the graft-host junction in our study suggests that the junction remains vulnerable, even following DALK, and can rupture with trauma. In our series, depending upon the severity of the trauma, postkeratoplastic WD can be associated with a good visual prognosis.


RESUMOObjetivo:Avaliar as características, os fatores de risco, resultados, e tratamento de pacientes que apresentaram deiscência traumática de sutura após transplante de córnea.Métodos:Estudo retrospectivo em que foram avaliados 11 olhos de 11 pacientes submetidos a transplante de córnea e que desenvolveram deiscência de sutura entre janeiro de 2004 e dezembro de 2012 no Hospital Oftalmológico de Brasília.Resultados:Oito (72,7%) pacientes eram homens e três mulheres. Seis (54,5%) pacientes foram submetidos a ceratoplastia lamelar anterior profunda (DALK) e 5 pacientes a ceratoplastia penetrante (PK). A média de idade dos pacientes no momento do trauma era de 31,1 anos. O tempo médio entre o transplante de córnea e a deiscência de sutura foi de 12,82 meses (variando de 3 a 33 meses). A melhor acuidade visual corrigida (AVCC) doa pacientes antes do trauma era de 20/60 (0,48 logMAR), e após o trauma era de 20/160 (0,90 logMAR) (P=0,15). Em um caso, a acuidade visual reduziu para sem percepção luminosa devido a descolamento de retina e posterior atrofia bulbar. O trauma ocular acidental e a queda da própria altura foram as principais causas de deiscência de sutura nos olhos transplantados.Conclusão:Pacientes previamente submetidos a transplante de córnea apresentam um risco prolongado de deiscência de sutura. A ruptura completa na junção doador-receptor no nosso estudo sugere que mesmo após um DALK a junção permanece vulnerável e pode romper com o trauma. Na nossa série, dependendo da severidade do trauma, a deiscência pós-ceratoplastia pode estar associada a um bom prognóstico visual.

ABSTRACTSuture granuloma is a mass forming benign lesion that develops at the site of surgery as a foreign body reaction to non-absorbable suture material. We present a case of suture granuloma that developed at the inguinal region after orchiectomy, and define the sonography, color Doppler sonography and real-time ultrasound elastography findings in correlation with the histopathological findings.

Introdução: O interesse antigo na reconstrução dos defeitos labiais pode ser explicado pela sua localização e pela singularidade da sua anatomia. Assim, o objetivo deste trabalho é discutir as melhores alternativas para a reconstrução labial. Métodos: Foi realizada uma análise retrospectiva de 50 casos submetidos à reconstrução primária dos lábios superior e inferior com o emprego de retalhos locais, no período de janeiro de 2000 a janeiro de 2014. Os defeitos foram divididos quanto à sua localização e quanto ao tamanho: defeitos que acometem até 1/3 do lábio e mais de 1/3 do lábio. O período de acompanhamento pós-operatório variou de 2 meses a 3 anos. Resultados: A idade dos pacientes variou entre 22 e 91 anos. A maior incidência foi na 7ª década de vida. Dentre as 50 reconstruções realizadas, 33 foram no lábio inferior (66%) e 17 no lábio superior (34%). Para defeitos pequenos, de até 1/3 do lábio, empregou-se a sutura simples (23 casos). Os demais 27 casos, cujos defeitos eram maiores que 1/3 do lábio, exigiram procedimentos com graus variáveis de complexidade. Conclusões: Para a reparação dos defeitos de até 1/3 do lábio inferior, a sutura direta é a melhor opção. Para a reconstrução de defeitos maiores que 1/3 do lábio inferior, devemos optar pelos retalhos de Gillies e Karapandzic, pois são confiáveis e capazes de restaurar a função labial. No caso de defeitos maiores que 50% do lábio, devemos evitar as técnicas de Gillies e Karapandzic, a fim de evitar a microstomia.


Introduction: Reconstruction of lip defects is popular owing to lip location and its anatomy. Thus, the objective of this work was to discuss the best options available for lip reconstruction. Methods: This retrospective study included 50 patients who had undergone primary upper and lower lip reconstruction using local flaps, between January 2000 and January 2014. The defects were divided according to their location and size: defects affecting up to 1/3 of the lip and affecting more than 1/3 of the lip. The postoperative follow-up varied from 2 months to 3 years. Results: Patient age ranged from 22 to 91 years. Most patients were in their seventies. Of the 50 reconstructions performed, 33 were in the lower lip (66%) and 17 in the upper lip (34%). For small defects affecting up to 1/3 of the lip, simple sutures were used (23 cases). The other 27 cases, in which more than 1/3 of the lip was affected, required interventions of different complexities. Conclusion: When repairing defects up to 1/3 of the lower lip, the direct suture is the most suitable option. For defects affecting more than 1/3 of the lower lip, the Gillies and the Karapandzic flaps should be chosen instead, as they are reliable and allow the reestablishment of lip functionality. For defects affecting >50% of the lip, and the Gillies and the Karapandzic flaps should not be considered, specifically to avoid microstomia.

ABSTRACT

A hemorragia pós-parto é uma complicação presente em mais de 18% dos nascimentos e é responsável por cerca de 25 a 30% das mortes maternas no mundo. Nas últimas décadas, várias técnicas conservadoras visando o controle hemorrágico no período pós-parto têm sido relatadas, com destaque para as ligaduras vasculares, suturas uterinas compressivas, embolização arterial, balões intrauterinos e a interrupção do ato operatório com laparostomia, empacotamento pélvico e posterior abordagem cirúrgica. O presente artigo apresenta uma revisão das suturas uterinas compressivas, com destaque para a cronologia de criação das técnicas de execução, e as publicações em periódicos. São descritas as suturas de Schnarwyler, B-Lynch, Cho, Hayman, Bhal, Pereira, Ouabha, Hackethal, Meydanli, Marasinghe-Condous, Matsubara-Yano, Zheng e a técnica de Halder. Foram detalhadas também as indicações, as técnicas associadas e as complicações.(AU)


Postpartum hemorrhage is present in over 18% of births and accounts for 25 to 30% of maternal deaths worldwide. In the last decades, several conservative techniques direct at controlling hemorrhage in the postpartum period have been reported, principally vascular ligatures, uterine compression sutures, arterial embolization, intrauterine balloons and surgery interruption with laparostomy, pelvic packing and subsequent surgical intervention. This article presents a review of uterine compression sutures, especially the chronology of creation of the execution techniques and publications in journals. Schnarwyler, B-Lynch, Cho, Hayman, Bhal, Pereira, Ouabha, Hackethal, Meydanli, Marasinghe-Condous, Matsubara-Yano, Zheng and Halder sutures were reviewed. Indications, associated techniques and complications were also detailed.(AU)

Introdução: O aprendizado de técnicas de sutura é um pilar importante na atuação do médico generalista, porém, muitas vezes o treinamento desses é limitado durante a graduação de medicina. Método: Através do Curso Teórico-Prático de Suturas, oferecido pela Liga e pelo Serviço de Cirurgia Plástica e Microcirurgia Reconstrutiva, são realizadas atividades teóricas-práticas com carga horária de 20 horas utilizando línguas de boi para treinamento de estudantes de medicina. Tem-se o objetivo neste trabalho de apresentar a rotina utilizada para fornecer embasamento teórico-prático na realização de procedimentos de sutura, como também os resultados obtidos que demonstram o aproveitamento do curso por parte de seus participantes. Resultado: Os alunos da 8°edição do curso de suturas foram avaliados através de questionários, obtendo conceito muito bom em quesitos como motivação e compreensão das aulas, relação teórico-prática e outros. Conclusão: O modelo deste curso de sutura mostrou-se como uma importante ferramenta na formação acadêmica, cumprindo os objetivos propostos.


Introduction: Learning suture techniques is a major pillar in the development of a general practitioner. However, training in these skills is often limited during under graduate medical education. Method: The League and the Plastic Surgery and Reconstructive Microsurgery Service offer a 20-hour course in Theoretical and Practical Suturing for medical students, using ox tongues for training. This study describes the approach to a theoretical and practical basis for suture procedures, and the resulting application by the participants. Result: Students in the eighth session of the suturing course were evaluated through questionnaires, yielding good information on issues such as motivation and comprehension, theoretical-practical relationships, and others. Conclusion: This suture course model was an important tool in academic education, and met the desired goals.

ObjectiveTo investigate the safety and feasibility of self-retaining bidirectional barbed absorbable suture application in retroperitoneoscopic partial nephrectomy.Materials and MethodsFrom Sep 2011 and Aug 2012, 76 cases of retroperitoneoscopic partial nephrectomy were performed at our hospital. The patients were divided into two groups: self-retaining barbed suture (SRBS) group (n = 36) and non-SRBS group (n = 40). There was no significant difference in age, sex, tumor size and location between the two groups. Clinical data and outcomes were analyzed retrospectively.ResultsAll 76 cases of retroperitoneoscopic partial nephrectomy were successfully performed, without conversion to open surgery or serious intraoperative complications. In the SRBS group, the suture time, warm ischemia time and operation blood loss were significantly shorter than that of non-SRBS group (p < 0.01), and operation time and hospital stay were shorter than that of non-SRBS group (p < 0.05).ConclusionsThe application of self-retaining bidirectional barbed absorbable suture in retroperitoneoscopic partial nephrectomy could shorten suture time and warm ischemia time, with good safety and feasibility, worthy of being used in clinic.

ABSTRACT

Objetivo: Evaluar a corto y mediano plazo una población de pacientes a quienes se les practicó sutura meniscal. Se analizaron específicamente las fallas procurando identificar las variables vinculadas. Materiales y métodos: En el periodo comprendido entre Enero de 2003 y Enero de 2011, fueron practicadas por el mismo equipo quirúrgico 1345 artroscopías simples y 785 reconstrucciones de LCA. En forma retrospectiva se seleccionaron los casos en los cuales fueron practicadas suturas meniscales (71 casos) y se identificaron las fallas (18 casos). Se definieron variables potencialmente vinculadas a las fallas como la edad del paciente en el momento de la reparación y al momento de la re-intervención por falla, el lapso transcurrido entre la primera intervención y la primera manisfestación clínica de la falla, el mecanismo traumático asociado, las características del compromiso meniscal, la comparación entre los patrones lesionales original y postsutura y la variedad de sutura realizada. Resultados: El porcentaje total de falla fue 25,35 por ciento, 18 de 71 reparaciones. En 13 casos el procedimiento asociado fue reconstrucción de LCA (porcentaje de falla: 18,30 por ciento) y en 5 pacientes se había realizado la sutura meniscal aislada (porcentaje de falla: 7,04 por ciento). El promedio de edad fue de 27 años (rango: 15-55). En todos se había realizado sutura “afuera-adentro” con Ethibond, PDS y Vicryl 1. En 4 casos se asoció sutura “todo adentro” con RapidLoc NR para los tercios posteriores. 11 pacientes (55,5 por ciento) refirieron un nuevo episodio traumático. El lapso transcurrido entre sutura y aparición de los síntomas fue: promedio 16 meses (3-45). Siendo el patrón lesional de la reruptura: en 9 casos lesiones longitudinal-vertical, en 6 c. lesiones en asas de balde y en 3 c. lesiones complejas. Conclusión: En la serie estudiada los factores más frecuentemente asociados con la re-ruptura fueron: paciente más joven, mecanismo traumático y patrón lesional longitudinal-vertical. La incidencia de fallas fue coincidente con la publicada a nivel internacional. Tipo de Estudio: Serie de casos - Estudio Terapéutico. Nivel de evidencia: IV

ABSTRACT

PURPOSE: To compare sutures with polypropylene and poliglecaprone 25 after partial cecotomy in rats. METHODS: Thirty six rats divided into two groups, A and B, of 18 animals; each group was also divided into three subgroups of six animals sacrificed at 4th, 7th and 14th days after surgery. Were studied the mortality, morbidity, complications attributable to sutures, macroscopy, optical microscopy and measurement of hydroxyproline at the level of the suture. RESULTS: There were no deaths or wound complications such as hematoma, seroma, abscess, evisceration or eventration. On microscopic evaluation reepithelization, coaptation and inflammation in both groups did not differ significantly. The average rate of tissue hydroxyproline found in the samples on the 4th day after surgery, was 21.38 mg/g tissue for group A and 16.68 mg/g for group B; on day 7 after surgery, the average was 15.64 mg/g tissue for group A and 26.53 mg/g for group B; on day 14, the average was 8.09 mg/g tissue for group A and 25.07 mg/g for group B. CONCLUSION: There were no differences on clinical evolution, macroscopic aspect, microscopic data and hydroxyproline concentration on both sutures.


OBJETIVO: Comparar a sutura com fio de polipropileno e poliglecaprone 25 após cecotomia parcial em ratos. MÉTODOS: Trinta e seis ratos foram distribuídos em dois grupos A e B de 18 animais, e cada grupo foi dividido em três subgrupos de seis, sacrificados no 4º, 7º e 14º dias do pós-operatório. Estudou-se a mortalidade, morbidade, complicações atribuíveis às suturas, macroscopia, microscopia ótica e dosagem de hidroxiprolina no nível da sutura. RESULTADOS: Não houve mortalidade ou complicações da ferida operatória como hematoma, seroma, abscesso, evisceração ou eventração. Na avaliação microscópica os critérios de re-epitelização, coaptação e processo inflamatório ambos os grupos não apresentaram diferença significativa. A taxa tecidual média da hidroxiprolina encontrada nas amostras no 4º dia de pós-operatório foi de 21,38 mg/g de tecido para o grupo A e 16,68 mg/g para o grupo B; no 7º dia a média foi de 15,64 mg/g de tecido para o grupo A e 26,53 mg/g para o grupo B; no 14º dia ela foi de 8,09 mg/g de tecido para o grupo A e 25,07 mg/g para o grupo B. CONCLUSÃO: Não houve diferença estatística entre a evolução clínica, avaliação macroscópica, microscopia e dosagem de hidroxiprolina entre as suturas realizadas com os fios estudados.

ABSTRACT

El objetivo del presente informe es describir un caso de queratopatía cristalina causada por microorganismos pertenecientes al grupo Streptococcus mitis en una paciente que concurrió a la consulta oftalmológica por molestias en su ojo derecho. Al examen oftalmológico presentó un punto de sutura interrumpida de nylon 10-0 sin tensión y con secreciones mucosas adheridas. El punto flojo fue retirado bajo normas de asepsia. Se indicó colirio de moxifloxacina al 0,5 %; el ojo tuvo una evolución adecuada, con una correcta epitelización. Sin embargo, luego de 15 días desarrolló un infiltrado blanquecino arboriforme. Se tomó una muestra en el quirófano, enhebrando el trayecto intraestromal de la sutura retirada con sutura de vicryl 7-0. Se indicaron colirios de vancomicina con 50 mg/ml. El infiltrado se mantuvo estable durante 45 días, luego se incrementó el tamaño y se produjo necrosis tisular con peligro de perforación corneal. Se realizó un recubrimiento conjuntival bipediculado. La paciente evolucionó favorablemente y luego de la retracción espontánea del recubrimiento, se observó leucoma cicatrizal y neovasos corneales.


Crystalline keratopathy: an infrequent corneal infection produced by the Streptococcus mitis group. The objective of this report is to describe a case of crystalline keratopathy caused by the Streptococcus mitis group corresponding to a patient who attended hospital for discomfort in his right eye. The ophthalmological examination showed an interrupted stitch of 10-0 nylon suture without tension and with attached mucus secretions. The loose suture was removed under aseptic conditions. Moxifloxacin 0.5 % eye drops were topically indicated. The treated eye successfully epithelialized and evolved favorably. However, after 15 days, a white tree-shaped infiltrate developed. A corneal sample was taken in the operating room, threading the intrastromal path of the removed stitch with a 7-0 vicryl suture. Vancomycin 50 mg/ml drops were indicated. The infiltrate, which was stable for 45 days, later increased its size and tissue necrosis occurred with danger of corneal perforation. A bipedicle conjunctival flap was performed in the affected corneal area, which evolved favorably. After spontaneous conjunctival flap retraction, only corneal scarring and neovascularization outside the visual axis were observed.
